In order to make your first visit with us as easy as possible, we provide below the forms you need to complete before your session begins. You can simply download each form (provided in PDF format) to your computer, print them, fill out the required information and bring them with you to our offices.

If you are a CPS client, all paperwork will need to be completed at, rather than before, your first visit.
